The Collaborative Care Teams aim to prevent avoidable admission to hospital and facilitate early discharge. We provide an MDT approach in patients own homes.
The Collaborative Care teams have been awarded the Airedale Partnership Award for working “creatively, enthusiastically & collaboratively to enhance our place-based Urgent Community Response model & offer to the population of Airedale, Wharfedale and Craven.”

This is why I absolutely love my job and am so proud of my Ward 6 team. This couple are both patients on our ward, she is very unwell and he wants to be by her side! So we moved them into sideroom so they could be together as they have been married 69 years!!💖 @AiredaleNHSFT

Our community teams understand the power of conversation. Sometimes they’re the only person that a patient sees all day – or all week. Talking reduces isolation and increases wellbeing #BDCTTimetoTalk@OurNeighbours@ACCTCCCT
📺 Jez, Sarah and the @ACCTCCCT team at @AiredaleNHSFT will be visiting Anthony at his home in the #YorkshireDales for the next few weeks to administer IV antibiotics & ensure he remains safe and well out of hospital.
What a difference this care closer to home makes! @nhsengland
