At the same time, the first guy having to be killed twice makes the best sense as an intentional set up for the interpretation that those were his first two kill experiences.

Stating that fact is not the same as claiming that eating with your hands causes diarrhea death. And it's relevant evidence even though it's not conclusive evidence of causation.
