“This situation, where over a million older patients are known to have endured prolonged periods of over 12 hours in EDs, frequently in unsuitable and undignified settings such as corridors, is a matter of grave concern” says SAM president Dr Nick Murch https://t.co/XAoD3GKzPD

“These findings, while shocking, are not unexpected. The issue of corridor care is one we, and many others, have raised consistently over recent years — not just months” - Dr Nick Murch, president of SAM, on @RCPhysicians snapshot survey on corridor care https://t.co/F9kPC8Uihm
💬 ‘As an urgent care doctor I see NHS at its worst – Starmer’s plan is deeply flawed’
🩺 Dr Tim Cooksley, a consultant in acute medicine in Manchester, tells @PMGallagher1 the Prime Minister risks an ‘eternal crisis’ in the NHS without urgent reform
https://t.co/BhMfaVk7RE
A real concern here is that when flu season settles people will feel major NHS crisis is over and emphasis for urgent action reduced.
It won't be. It isn't the fundamental cause of current emergency.

Another winter of discontent looms for NHS urgent and emergency care https://t.co/VkDJUawrJp
As the crisis deepens, the solutions take more time and resources to implement; neither of which are politically desirable. However, this is the stark reality that needs to be accepted.
As Keir Starmer says we are all desperate for a plan the NHS can unite behind. This has some important elements; but for many in NHS this is not it. A coherent co-ordinated whole-system approach, including social care, that focuses on increasing capacity and workforce is required
"The media is full of pictures of patients in corridors; long queues of ambulances outside EDs and stories of horrific patient experiences: this is seen and experienced by frontline staff every day. It is an appalling situation.
@doctor_oxford@ShaunLintern
"Despite the positive rhetoric around tackling waiting lists, our concern is that the government continues to avoid acknowledging overtly that there will be no elective recovery without emergency care recovery” says SAM immediate past president @acutemed2 https://t.co/B2cozlptrc
There is insufficient workforce and capacity to meet the demands of an increasingly ageing population...with simply no resilience to cope with any excess strain. The sooner the Government acknowledges this reality and need to find social care - the better for everyone @acutemed2
"The extra cases of winter viruses... mean many hospitals will fall into critical incidents and patient safety will be compromised - not as result of hard working staff not putting it first but because hospitals simply cannot cope" @acutemed2 tells @theipaper
SAM Pres Nick Murch - uptick in flu cases already being "felt in urgent and emergency care where delayed assessment...and degrading corridor care is continuing" @RCollEM@RCEMpresident
https://t.co/WkwOGO1o8Q
SAM have launched their SAM survey. PLEASE can all people who work the majority of their clinical time in acute medicine complete this - you do NOT have to be a SAM member. https://t.co/iiuCgwP32k
At the heart of this appalling new normal is a system where demand continues to exceed capacity. No hospital can ‘play ball’ whilst functioning at 100% capacity
We are hurtling into months of chaos as this winter will be a new terrifying nadir @acutemed2

Antihypertensive therapy in hypertensive ED headache patients is associated with *increased* hospital admission AND ED length of stay, but *no* difference in headache pain levels

Many agree with diagnosis.
At heart demand exceeded capacity for too long. Investment at heart of any recovery. No sign that on way.
Without situation will remain dire. Degrading care and harm to patients.
Blame will soon shift.

Dr Nick Murch, president of the Society for Acute Medicine, said the new NHS figures "demonstrate the scale of the short-term challenge"

Time to stop the term 'Virtual Wards' - patients and their families find it confusing (as do clinicians).
Hospital at Home describes what we do, links to a global evidence base and a world wide network of clinicians, researchers and societies who all use 'Hospital at Home'
