Pain is not the disease itself; it is a symptom generated by conditions like Acute Coronary Syndrome, inflammation, or tissue injury. If we leave pain untreated, it increases stress hormones, raises heart rate and blood pressure, and can actually worsen the patient’s condition.

We give analgesics not to hide the problem, but to:

➡️ Reduce physiological stress
➡️ Improve breathing, mobility, and recovery
➡️ Allow proper examination and treatment
➡️ Prevent pain induced complications

Treating pain does not delay diagnosis, it improves patient stability while we treat the cause.

Patient comes in with hematemesis / melena → think Upper Gastrointestinal Bleeding👇

Step 1: Don’t panic, resuscitate first

2 wide-bore IV lines
Blood for CBC, PT/INR, crossmatch
Start fluids / blood if needed
Oxygen if unstable

Step 2: Protect the patient

Early PPI (e.g. IV pantoprazole)
If variceal bleed suspected → start octreotide + antibiotics

Step 3: Decide severity

Shock? tachycardia? falling Hb? → ICU/HDU
Otherwise close monitoring

Step 4: Definitive step = endoscopy

Ideally within 24 hours (earlier if unstable)
Treat source: banding / clips / injection

Key reminder:
👉 Resuscitation always comes before endoscopy.
