A patient kept being treated for recurrent treatment failure

Pharmacist sat with the patient and discovered they had been taking methotrexate DAILY instead of WEEKLY for weeks. Immediate intervention prevented catastrophic toxicity. Pharmacy is beyond dispensing indeed!

No! You should not open or remove the outer layer of most medicines unless a pharmacist or medical doctor specifically tells you to.

Those capsules or “outer layers” people see are not rubber or plastic. They are usually made from safe materials like gelatin or special pharmaceutical coatings, and they are added for an important reason. That outer layer is not harmful and it’s actually part of the safety and effectiveness of the medicine.

Think of a medicine tablet like a carefully designed delivery system, not just a lump of powder.

That outer layer can do different jobs:
• Sometimes it protects the medicine so stomach acid does not destroy it
• Sometimes it protects the stomach from irritation
• Sometimes it controls how fast the drug is released in the body
• Sometimes it helps the medicine stay stable until it reaches where it needs to work
So when someone opens a capsule or crushes a tablet, they are not just “removing a layer”…they may be breaking how the medicine was designed to work.

That can cause problems like:
•the medicine not working properly
•the drug being released too fast
•more side effects than expected
•or the medicine becoming less effective

Simple rule:
If a medicine is not clearly labelled as chewable, dispersible, or safe to open, it should be swallowed whole.That outer layer is not harmful. It is actually part of the safety and effectiveness of the medicine.

Pharmacists continue to receive market premiums benchmarked against a 2012 basic salary, despite the profound economic shifts and the sustained rise in cost of living over the past 14 years. This growing gap between remuneration and present-day realities is becoming increasingly difficult to overlook and may require thoughtful review.
