@garrettsbarry @thanh_neville Almost every (x) is better than (y) statement is a generalization that isn’t helpful. I get called an unsustainable amount to place PIVs in floor patients. Nurses aren’t getting taught, nor are most docs. I try and teach US when I go to help and I’m usually dismissed.

@Ana__George @fuzzymittens For context @Ana__George the “time to peak effect” for these specifically designed fentanyl patches is around 10-12hrs. Meaning it takes 12hr to reach peak after applying it to the skin (with a patch that took years and millions of R&D $$ to design)

@DrAdamBitterman Sometimes surgery is hard and takes longer than expected, same for anesthesia start. Most of my starts are very quick but listening to people whine about cut time is annoying when a med student takes 40 mins to close a lap and the staff has already scrubbed out and gone home.

All those times you wonder why something is done a certain a way and don’t ask will add up. It’s like not knowing someone’s name, it gets harder and harder to excuse the longer you know them. Then youre in final year and have basic holes in knowledge. Ask Qs!

@JedBarash @doctornontrad As someone spending more time with Barash than my wife at the moment I definitely agree to an extent. However there’s no rush to get into Barash. Lange is a more approachable start as a junior learner in my opinion. Love me some Barash tho. Also hate it.

@DrShaena I put epidurals in for a living (among other things). Often 10+ a night. While making 22 grand a pop sounds lovely, it is sickening that is what pt is charged. Sickening. What a mess. I’m so sorry you’re going through this.

@fuzzymittens The doctrine of double effect!
As a bolus this seems aggressive unless the patient was HOT. I use our ICU ppo, and Rx a moderate dose, with a short interval, and no upper limit. I empower the nurses to dose as they see fit within ppo to ensure the patient is comfortable.
