Why does this matter?

Among patients exposed to unreliable testing, we observed downstream consequences :

• Repeat testing
• Thrombophilia labeling
• Pregnancy-related treatment
• Lifelong anticoagulation

Diagnostic errors can extend far beyond the laboratory result

•22.4% of thrombophilia tests were performed under biologically unreliable conditions.

• Common unreliable testing settings included recent thrombosis, pregnancy, heparin exposure, and warfarin therapy.

IVC filter quality-improvement project led to :
appropriate insertions increased from 73% to 87%, and retrieval rates from 76% to 95% (both statistically significant).
Using policy restriction, automatic hematology consultation, weekly rounds, and targeted education.
