🔹 Newly diagnosed active PMR
→ Start prednisone 12.5–25 mg/day (or equivalent)

🔹 High risk of glucocorticoid toxicity?
→ Consider sarilumab or tocilizumab
→ Methotrexate if IL-6 inhibitors aren’t suitable

🔹 If remission achieved
→ Gradual steroid taper
(typically to 10 mg within 4–6 weeks, then ↓ by ~1 mg every 4 weeks)

🔹 If remission not achieved / relapse
→ Increase glucocorticoids
→ Consider starting or switching targeted therapy
