The family of 54-year-old Pat Mthembu is demanding answers after his death on a farm in Driefontein, Isinembe, on Saturday, claiming the circumstances surrounding the incident are suspicious.
According to relatives, Mthembu was called from his home to assist with a tractor. At approximately 13:15, the family received a phone call informing them that he had died. They were also sent photographs from Mthembu's own cellphone, which they say was protected by a unique password known only to him.
Family members told Reaction Unit South Africa (RUSA) that Mthembu was not a mechanic and questioned why he had been called out to work on the tractor.
When relatives arrived at the scene, they allegedly found that the tractor was no longer there. However, a jack remained in place and appeared to have been lowered over his head. They also claim photographs showed blood on his boots.
RUSA confirmed that the family contacted the organisation on Sunday regarding the incident.
The South African Police Service (SAPS) has not yet commented on the cause of death. The circumstances surrounding the incident remain under investigation. This version keeps the allegations clearly attributed to the family and avoids presenting unverified claims as established fact.

His reporting has exposed issues affecting Alexandra residents, including concerns about food safety in local shops, allegations of animals being kept in unhygienic conditions in businesses, and the struggles of vulnerable families who need assistance to bury their loved ones.
