The fire imagery and motif Makaveli used in "Smile" is brilliant.

"Backstabbed and bleedin'
Crooked thoughts laced with weed, learnin'

Will I be smothered by my own pain?"

He used the words "weed," "hot," "burnin'," "sherm," "smoke," "smothered," and "flames" because they're all connected to fire.

Obviously, weed is related to fire because that's what's needed to light the blunt or papers it's rolled in. Fire is obviously hot, and it burns. A sherm blunt, joint, or cigarette needs to be lit by fire to smoke it. Fire is what causes smoke. Flame is, of course, the glowing gaseous part of a fire, and it can be smothered; the smoke created by fires can also smother people, and the smoke from sherm can smother your lungs. Sherm smoke can also cause physical pain to the lungs. When he says, "will I be smothered by my own pain," he's metaphorically comparing his pain to smoke from fire, and it's no coincidence that he said this right after talking about inhaling sherm smoke. The two lines are adjacent by design.

When he says to "visualize the flames," he's not only saying that's what he himself visualizes, he's also telling the listeners and readers to visualize the flames as well. He's referring to the flame at the tip of a sherm blunt, cigarette, or joint, but he's also referring to the flames in the biblical lake of fire and the figurative flames on earth, hence hell on earth. The word inhale is being used as a homophone here.

When he says, "bullets be hot, they burning, inhale the sherm smoke..." he's saying that the hot bullets are burning people "in hell," referring to hell on Earth. This is also a play on the "burn in hell" phrase. He also segues into mentioning that he literally inhales sherm smoke. The transition is seamless because he wrote it in a way that made two different things occur simultaneously. This was all by design. He had been shot before, so he knew from first-hand experience how badly bullets burned.

Sidenote: He's not saying that he was stabbed in the back with a knife literally, but he used the word "bleedin'" because people bleed when they literally get stabbed in the back, and he was actually backstabbed and shot in real life. He also smoked a blunt at the scene of the shooting at Quad Studios; he's referring to the crooked thoughts laced with weed that were in his mind in that very moment. Some say experience is the best teacher, and Pac certainly learned some things from that experience. Hence, his use of the word "learning." It's also general, and not just pertaining to what I just previously mentioned. Long live the greatest poet and lyricist to ever exist.
