صرح المتحدث الرسمي باسم وزارة الدفاع اللواء الركن تركي المالكي أنه إلحاقاً للبيانين الصادرين من وزارة الدفاع يومي الاثنين (١٣ صفر ١٤٤٨هـ) والثلاثاء ( ١٤ صفر ١٤٤٨هـ) الموافقين ( ٢٧- ٢٨ يوليو ٢٠٢٦م) من أن الدفاعات الجوية اعترضت ودمرت عدداً من المسيّرات التي حاولت استهداف منشآت بترولية في منطقتي الشرقية والرياض، وأن هذه الاعتداءات الإرهابية انطلقت من الأراضي العراقية ونفذتها ميليشيات إرهابية موالية لإيران، وما تم التأكيد عليه من حق المملكة الأصيل في الدفاع عن نفسها ومقدراتها واحتفاظها بحق الرد في الوقت والمكان المناسبين ومصداقاً لقوله تعالى: (فمن اعتدى عليكم فاعتدوا عليه بمثل ما اعتدى عليكم)، وانطلاقاً من حق الدفاع عن النفس الذي يكفله القانون الدولي وفق المادة (٥١) من ميثاق الأمم المتحدة، قامت القوات المسلحة السعودية بالتنسيق مع القيادة المركزية الوسطى الأمريكية هذا اليوم الأربعاء (١٥ صفر ١٤٤٨هـ) الموافق (٢٩ يوليو ٢٠٢٦م) بشن ضربات نوعية محددة ضد أهداف تابعة لتلك الميليشيات المتواجدة على أراضي جمهورية العراق والمرتبطة بالاستهدافات على المنشآت البترولية في المملكة.
وتؤكد المملكة على أنها لا تسعى إلى التصعيد إلا أنها سترد على أي عدوان تتعرض له.

Although linezolid is a reversible MAO inhibitor, the largest population-based Canadian cohort (JAMA Network Open, 2022; n=1,134) found serotonin syndrome to be exceptionally rare (<0.5%), with no statistically significant increase in risk among patients receiving antidepressants. Risk assessment, clinical monitoring, and evidence based decision making should always take precedence over theoretical concerns.

Understanding MICs and CLSI breakpoints is one of the most important skills in infectious diseases and antimicrobial stewardship.

A culture report should never stop at:
“Susceptible.”

Instead, we should ask:
• What is the MIC?
• Is it close to the breakpoint?
• Is the dosing optimized?
• Does the patient’s condition affect clinical success?

A great reminder that not all “susceptible” results are equally strong clinically.

Steroidal MRAs (spironolactone, eplerenone) vs Non-steroidal MRAs (finerenone, balcinrenone, aparenone, esaxerenone)
Key differences:
🔵 Steroidal MRAs:
• Higher binding in kidneys vs heart
• More off-target binding → increased side effects (gynecomastia, hyperkalemia)
• Clinical data: benefit in HFrEF ✅ | Role in HFpEF unclear ❓
🟠 Non-steroidal MRAs:
• More even distribution between heart and kidneys
• More potent MR antagonism
• Clinical data: Role in HFrEF unclear ❓ | Benefit in HFpEF ✅
The pharmacological differences are not trivial — tissue distribution, specificity, potency, and tolerability all differ.
Fascinating inversion: sMRAs work in HFrEF, ns-MRAs work in HFpEF. Same target, different patients, different drugs.
The MRA class is not one-size-fits-all.

So, why did the guidelines say to county Day 1 with neg. blood cultures? Because the RCT of ampho vs. flucon to treat diss. candid. was designed with a 14 day course of therapy, from first day of neg blood cultures. PMID 12746765, published in 2003, during the guideline process.
