This is worth further debate. C9761 is a procedure code defined by use of a steerable vacuum aspiration catheter (distinct from 52356).
Steerable: Merriam-Webster defines “steerable” as “capable of being guided” and “navigable” as “capable of being steered.” The terms are essentially interchangeable; the code does not require an independent steering mechanism.
Vacuum aspiration: Vacuum and suction are clinically the same (both rely on suction force from a vacuum source).
Catheter: All FANS sheaths meet the AAPC definition of a catheter (flexible tube inserted into a vessel or urethra for instruments/fluids). Some manufacturers even label them as catheters in their IFU.
The referenced 2024 Urology Times article claiming C9761 "would not be appropriate for FANS" is outdated. In a 2025 podcast, co-author Mark Painter stated that both FANS and CVAC fit the C9761 definition, though payers should still be verified.
FANS clearly performs the described procedure
I read the 420-page SIU-ICUD 2026 Consultation on Male LUTS carefully. It deserves an equally careful critical reading.
Full disclosure: 10,000+ HoLEP, 600+ urologists trained, commercial ties with HoLEP manufacturers. I'm not neutral — but neither is this document.
Key findings:
→ Every sponsor's product has a dedicated chapter. PROCEPT funds the document; the WATER trial PI co-chairs the Aquablation committee.
→ 4 corporate PROCEPT images appear as "scientific figures." No other procedure gets this.
→ HoLEP: Level I, Grade A — restricted to >80 mL with NO justification. EAU imposes no such limit.
→ Aquablation: Level II, Grade B — gets a procedural video. HoLEP gets none.
→ Zero individual COI declarations in 420 pages.
