I was on call in a private hospital when one of my nurses developed abdominal pains.
I ran around despite resistance from the hospital's admin to ensure she got an Ultrasound (they wanted her to pay first!).
Results showed Acute Appendicitis. This private hospital refused to book her in for urgent surgery until she made a deposit of 750K (Ps-She works here and her salary is less than 180K).
We mobilized her and sent her with another nurse to a government hospital. She went to 4 different government hospitals and 'No bed space' was all they said and turned them back (not even minding she was a nurse!).
I had to ask them to bring her back to the private hospital where she finally had the surgery after myself and 3 other nurses including the patient signed to have deductions from our salaries till the debt was paid.
She had a ruptured appendix when we finally went in which changed the surgery from a simple appendectomy to an Ex-Lap. 750k became 1.6M and we had to pay it for 5 months till it was completed. She is fine now but just putting this out here.

UPDATE TO ALL MEMBERS ON WORK HOUR REGULATION (AND EXCESS WORKLOAD)

Good evening Distinguished NARDITES!

The leadership of NARD ably led by the Strategic P-MUS (Garkuwan Matasan Arewa), is pleased to update all members on a significant development arising from the deliberations of the FMoHSW Ministerial Committee on Work Hour Regulation and Casualization of Doctors.

Following extensive engagements and a comprehensive nationwide situational assessment involving 6,350 healthcare personnel across the six geopolitical zones, the Committee has developed a National Policy on Work Hour Regulation for Health Institutions in Nigeria.

The findings clearly demonstrated the urgent need to address excessive work hours, burnout, fatigue, inadequate rest, and the associated risks to healthcare workers and patient safety.

NEWLY RECOMMENDED WORK HOUR GUIDELINES

The following key provisions have been recommended under the new framework:

1. Maximum Weekly Work Hours:
Healthcare personnel shall work a maximum of 48 hours per week, including weekends. Any authorized excess hours shall be appropriately compensated.

2. Maximum Shift Duration:

- Day shift: 8 hours maximum

- Afternoon shift: 6 hours maximum

- Extended shifts exceeding 12 hours shall be limited to emergency situations only.

- All authorized excess hours must be adequately compensated.

3. Maximum Call Duty:

- A maximum of two call duties per week, including weekend calls.

- Calls must be equitably distributed among eligible personnel.

- A maximum of one weekend call per month.

4. Maximum Consecutive Duty Period:
There shall be an absolute maximum of 24 continuous hours of duty, inclusive of call duty.

5. Mandatory Rest Periods:

- A minimum of 12 hours between regular shifts.

- Mandatory 24-hour post-call rest following weekday calls, Saturday calls, Sunday calls, and public holiday calls.

6. Mandatory Break During Duty:
A minimum one-hour break after six hours of continuous work, including shift and call duties, shall be provided. Breaks should be appropriately staggered to ensure uninterrupted patient care.

7. Equitable On-Call Distribution:
Call duties shall not exceed two per week and one weekend call per month, with equitable distribution among eligible healthcare personnel.

8. Compensation for Excess Work Hours:
Authorized excess work hours must be properly documented and adequately compensated above the normal hourly rate, or appropriately balanced through structured time-off. This provision must not be abused by institutions.

9. Protected Research and Development Time:
Healthcare personnel in all institutions should have protected time for research and professional development, subject to approval by the appropriate National or Institutional Health Research and Ethics Committee.

IMPORTANT NOTE:

These recommendations are without prejudice to the stipulated maximum shift, call and rest durations, particularly in genuine emergency situations where patient safety and continuity of care may require exceptional arrangements.

We are pleased to inform all members that these recommended guidelines have been forwarded to all MDs/CMDs for implementation across all health institutions.

Hospital Managements and Heads of Departments are expected to take appropriate steps towards full compliance of these guidelines.

We strongly encourage all Centre Leaderships to engage their respective Managements and follow up on the implementation of these guidelines.

Members are encouraged to remain vigilant, document cases of non-compliance and channel legitimate concerns through their center leadership for necessary action by NARD.

We await the release of the second document addressing casualization and abusive locum appointments soon.

Sarki, I met my wife when she was just a medical student with nothing but big dreams and an unwavering determination to become a doctor.

I stayed by her side through the sleepless nights, endless exams, clinical postings, house job, and NYSC.

We got married while she was a Medical officer, and today she’s a Junior resident in Internal Medicine, working tirelessly toward becoming a senior resident next year inshallah.

Before we got married, people warned me.

“Don’t marry a doctor.”

“She’ll never have time for you.”

“You’ll be lonely.”

“Your children will be raised by housemaids.”

“Her career will always come before her family.”

Wallahi, every single one of those predictions has been proven wrong.

Since the day we got married, I have never felt neglected not for one day.

I have never had to compete with her career for her love.

Despite the exhausting calls, emergency shifts, sleepless nights, and the emotional weight that comes with saving lives, she still comes home and gives her best to us.

She is an amazing doctor to her patients, an incredible mother to our daughter, and the most loving wife I could ever ask for.

She makes our house feel like a home. She never makes me feel like I’m carrying this marriage alone, I’ve always encouraged her to fly higher.

The problem has never been working women. The problem is most people assuming that every successful working woman will neglect her home.

My wife is living proof that a woman can wear a white coat all day, save lives, chase her dreams, and still come home to love her husband and children with her whole heart.

If I had to choose a wife a thousand times over, I would still choose her every single time.

It takes a commissioned officer in the Nigerian Army a minimum of 25 to 30 years of active, uninterrupted service to reach the rank of Major General. 30+ active years of service to his country, but look at how the system neglected him in those hard times.

This is someone that dedicated his whole life in service to Nigeria, who contributed more than those currently in Government, whose only contribution is looting our resources and buying their ways to power.
