Five key facts everyone should know about the @AmerAcadPeds 2018 policy statement on “gender affirming care” for kids:
1. As explained in the document, the policy was conceptualized, written, revised, and approved by a single author.
2. The author, Dr. Jason Rafferty, was then still in his residency.
3. The document explicitly says that it “does not… serve as a standard of care.” But that is how it has been understood by many in the medical community, and the AAP has done virtually nothing to dispel that impression. See, e.g., amicus briefs filed by medical groups on behalf of plaintiffs in lawsuits challenging state age restrictions.
4. Rafferty agreed to be “accountable” for the document’s contents, but neither he nor the AAP have ever responded to the authoritative, peer-reviewed critique by Dr. James Cantor: https://t.co/7FjiLNAigD
5. The 2018 statement uses a method called a “narrative review,” which in evidence-based medicine is considered far less reliable than a systematic review due to its vulnerability to author bias (cherry picking of studies and failure to conduct rigorous evaluation of the strength of the evidence).
