Part 3️⃣ -- Small But Mighty — Understanding the Role and Fragility of the UCL 🩺

If you’ve ever wondered how a pitcher’s arm withstands the incredible forces of a 100-mph fastball, the answer lies in a tiny ligament deep within the elbow: the ulnar collateral ligament (UCL). Measuring just 1.5–2 cm in length and as thin as three stacked credit cards, the UCL is the unsung hero of the throwing arm. But here’s the catch: despite its crucial role, the UCL is surprisingly fragile. Every throw places immense stress on this tiny structure, making it one of the most injury-prone ligaments in sports. In this post, we’ll explore the anatomy and biomechanics of the UCL, why it’s so susceptible to injury, and what that means for the future of throwing athletes. ⚾️💥

The Snapping Ulnar Nerve / Ulnar Nerve Transposition - Dr. Ahmad's Surgical Approach 🔪

Acute #elbow #pain with a snap sensation in a #pitcher is always concerning for a UCL injury and possible need for Tommy John surgery. But another less common injury that has a similar presentation is acute instability or snapping of the ulnar nerve 💪

The violent forces involved with #throwing can dislocate the ulnar nerve from its groove. When the nerve dislocates, it creates the painful pop sensation in the inside part of the elbow. Once dislocated, the tissues surrounding the nerve are stretched and the result is persistent snapping where the nerve pops in and out of the groove with throwing and can even happen with less forceful elbow motions. Dysfunction of the nerve from the repeated dislocations can result in numbness and tingling in the pinky and ring finger and occasionally weakness in the hand #muscles 🤚

Most commonly the snapping of the ulnar nerve is resolved with a surgical procedure called ulnar nerve transposition. In this procedure, the ulnar nerve is positioned in front of the #arm and stabilized using either subcutaneous tissues or a dense tissue adjacent to the nerve called the intermuscular septum.

Recovery time for this surgery depends on the degree of injury to the ulnar nerve. Throwing can be resumed as early as six weeks in many cases ⏰

Velocity, high intensity throwing, is something we can’t keep up with from an injury perspective. There’s so much emphasis on hard throwing. It used to be that year-round baseball was the issue and players would not take a break. Now, it’s the hunt for velocity with velocity-enhanced programs. During games, high intensity throwing with every throw. There’s no easy pitch anymore ⚾️❗️

A Survey of Healthy Players --- While #overuse #injuries like Tommy John are well-known in the #MLB, a lesser-discussed but equally troubling issue is arm pain in youth baseball players. 🚨⚾

🔍 A comprehensive study has delved into not just the presence but the impact of arm pain among these young athletes, revealing that a majority experience some level of pain that can dampen their enjoyment and performance. 📉

Let’s zoom in a little closer:

● 23% of youth players reported prior overuse injuries.
● Only 26% never experience arm pain when throwing, indicating widespread issues.
● Arm pain affected not just physical performance but also the psychosocial well-being of players, making baseball less fun for 30% of those affected.
● 46% were encouraged to play despite pain, highlighting a concerning trend in youth sports culture.
