However, there was obviously some poor communication, if they didn’t realise there was cardiology cover. We need to teach how to learn from incidents and excellence.

However, incident reporting systems need refining (there are some systems which are better and more nuanced than Datix) and to not be about blame, but allow proper ways to address and learn from the incidents.
