Let's talk ethics! 🗒️

Making a diagnosis online without having the full history or examination is unethical (cutting someone open without their consent is also unethical L surgeon moment). Many times, a diagnosis requires a blend of clinical reasoning and criteria from an HPI, exam, and testing.

Sometimes there are also clinical diagnoses that can be made without some of the above, where a few positive findings are enough to support the diagnosis even without others! These include pneumonia, heart failure, infective endocarditis, most psychiatric conditions, etc. This allows telehealth and remote prescriptions to be a thing.

But for most conditions, a full workup is gonna include a history of the present illness, a past medical history, family history, social history, review of systems, physical exam, vital signs, labs, etc. Without these things, I would be unable to differentiate chest pain from an MI or reflux, for example. No matter how many questions I ask, I'm gonna need to do a physical exam, get an EKG, draw troponins, before I can say one or the other. If I just diagnosed from the interview alone, there's a pretty big chance that I could be wrong; and an incorrect treatment call can sometimes be the difference between life and death. So even if the patient requests their own diagnosis based on this limited information, that is still unethical to offer given the consequences.

@saikenMD This is such an important thing to say. As a veterinarian I constantly have to explain this when people ask me questions about their pets, whom I’ve never seen, and I’m often met with the same misunderstanding. Thank you for always providing such clear and valuable information!

@Vincentgraphart I agree! What I love about DH is that since each class is made of two domains, I immediately start imagining new classes around domains that aren’t connected yet. A death cleric would be perfect for Splendor and Dread (but making a homebrew class isn’t ideal for new players lol)
