Let me speak directly to every Nigerian doctor reading this. Especially the young ones in residency, NYSC postings, and the early years of a career built on a decade of sacrifice.

Your clinical judgment is sacred. The moment financial pressure or a senior colleague’s impatience overrides what your training tells you about a patient, you have broken the most important contract between a doctor and the society that trained them. Patients come to you at their most vulnerable. That vulnerability is not a transaction. It is a trust the entire profession depends on.

Document everything. Every clinical decision. Every instruction from a superior you disagreed with professionally. Nigerian hospitals are full of cases where the junior doctor followed a wrong order and carried the consequence alone. Write it down. Date it. Keep copies. Your documentation is your protection in a system that will not always protect you.

On Japa, nobody has the right to judge that decision. But if you go, go to grow. Build subspecialties and technical competencies that make you genuinely world class, not just comfortably employed abroad.

On money, nobody teaches Nigerian doctors personal finance and it shows. Your peak earning years are roughly between 35 and 55. What you build financially inside that window determines the rest of your life. Start investing early. Do not wait until you are a consultant before your money starts working.

The Nigerian healthcare system is broken in ways that are not your fault. But within that broken system there are patients who will live or die based on whether the doctor standing over them that day brought their full self to work.

Be that doctor.

The untold tale of Nigerian health workers

Nonso graduated, paid for his license, gets licensed to practice and finally started practicing. It’s almost like dream come true.

However, two months into the grind, he realized that this is not the life he envisioned. Though he’s passionate about his job, the job now feels like passion of Nonso. So what’s the way out?

He starts researching and discovers that his profession is not entirely the problem, rather geography is making a mess of him.

He found out that his colleagues miles away can afford bread without breaking their back, whereas he would need to book an orthopedics appointment before going to the bakery here.

So he begins making arrangement, starving and saving every kobo to be able to pay for licensing exams in another country.

My guy is working hard and losing sweat every night and day at work and at the end of the month, the federal government pays him N270k for a job well done. Quickly, he converts this to USD and it’s equivalent to $175, meanwhile he needs roughly $1350 just to start the “japa” process.

God bless the federal republic of Nigeria

Now it has dawned on him that he needs to lock unholy in, so he starts doing double shifts; he’s active in all the locum groups in the six geopolitical zones.

I sometimes wonder what his passion really is: is it making an impact or just being able to afford bread?

One year down the line, he raises about $1.3k and starts the process, hoping to meet his luck someday.

But wait… you just carried your one-year savings and sent to someone miles across the sea, someone you’ve never met before?

That almost feels like yahoo in reverse to me💀

Few months or years down the line, he got his license, his second license!

Congratulations 🎉

Now it’s time for him to battle international economies and immigration laws from Afikpo.

Today, I remember this kid during my HJ in Ogbomosho.
Went to pluck mango by climbing a tree. Fell down head first. Brought in unconscious. I was HO for surgical team on call. We had no operation CT in LTH ogbomosho. Besides, at that time the only Neurosurgery capable center was in Ilorin.
To be fair, LTH Osogbo has a Neurosurgeon, Dr Rabiu but Osogbo was 1hr from Ogbomoso even though it's the same center. UITH was 35mins away. So away we went...I followed the Ambulance.
First stop was HADC where we got a CT showing Massive SDH.
Then sirened our way to UITH.
Got this chap there. They prepped for theatre and he died on the way to the theatre.

I wonder what could have happened if we had functional theatre, man power, equipments on site.

But that's nothing, the minister of power wants to buy air ambulance, so everything will be fine henceforth
