As Senior Civil Servant, Simon Hunter could potentially face prosecution for offences such as misconduct in public office or, gross negligence manslaughter.
That means wilfully neglect their duty to such a degree that it amounts to an abuse of the public's trust.

Campaigner for Reform of the Child Maintenance Service, Noel Wilcox, and victim of the service, John Hamond, reveal how thousands of parents have been allegedly overcharged on maintenance payments.
@GBNEWS In total he won 3 cases. A tribunal case in 2012. The CSA case closed in 2013.
7 years later the CMS use erroneous data to seize £19,269.09 of which his ex-partner knew nothing about. The CMS later conceded and signed a court order.
Then they went for a Liability Order and lost

but they are not very good with numbers - The truth is
40,000 were in households with a statutory CMS arrangement, while 80,000 were private arrangements.

If he can't get that data correct then what else is wrong?
