...a bony prominence caused by damage to the underlying soft tissues. It can develop before any open wound appears.
Signs to watch for:
โข Skin that appears deep red, purple or maroon
โข Areas that feel firm, boggy, or warmer/cooler than surrounding skin

Does your team do safety rounds or bullet rounds? Consider adding pressure injuries (PIs) to the agenda! Highlighting patients at high risk for PIs or identifying when patients have new risk factors can help facilitate interventions to support PI prevention and treatment.
Does your client have a pressure injury? A dietitian can help make sure that your client is consuming adequate calories, nutrients, fluids, vitamins and minerals to support tissue growth and skin integrity.
