Home care teams are using this framework to strengthen practice and improve outcomes:
✔ Clear, consistent expectations for skills, knowledge, and attitudes support better care delivery
✔ Self-assessment tools help providers reflect, identify gaps, and guide targeted learning
✔ Improvements in care planning, team communication, and early goals-of-care conversations
✔ Shift in understanding palliative care as a focus on quality of life - throughout the illness journey
✔ Increased confidence among personal support workers and nurses to advocate and respond to patient needs
This shared foundation is helping reduce variability and build more equitable, timely, and compassionate home-based palliative care.
