JDM UPDATE 2026: BEYOND CK — LOOK AT THE VESSELS!
Two new 2026 studies reinforce a key concept:
👉 Juvenile dermatomyositis (JDM) is a systemic vasculopathy—and its microvasculature may become a measurable biomarker.
🔬 NAILFOLD CAPILLAROSCOPY
In JDM/overlap myositis:
• Branched capillaries enriched in JDM/OM
• 4/5 TIF1γ+ JDM clustered in the branched phenotype
• Microhaemorrhage density correlated with:
PGA r=0.60
LDH r=0.43
vWF-Ag r=0.51
Neopterin r=0.69
📉 After treatment → ↓ microhaemorrhages + ↑ capillary density
🤖 Automated neural-network analysis may help transform NVC from a descriptive test → quantifiable imaging biomarker.
🧪 BEYOND CK: CXCL10 & GALECTIN-9
A second 2026 study (52 JDM) found 8/16 endothelial mediators elevated.
⭐ CXCL10 → PGA r=0.76
⭐ Galectin-9 → PGA r=0.65
Both also tracked nailfold microvascular injury.
CXCL10, Gal-9, Ang-2 & sTNF-RII differed between NXP2+ and TIF1γ+ JDM, suggesting MSA-specific vascular biology.
📌 2026 takeaway
JDM monitoring may be moving from:
CK alone ➡ phenotype + MSA + endothelial biomarkers + quantitative capillaroscopy
The nailfold may be telling us what is happening in the disease before muscle enzymes tell the whole story.

Oral vitamin B3 (nicotinamide) at 500 mg twice daily for 14 days improved nailfold videocapillaroscopy (NVC) patterns in around 45% of participants with primary or scleroderma-related Raynaud phenomenon (RP), with increased normal capillaries and reduced abnormal features. Long-term follow-up of patients with early Raynaud's phenomenon (RP) over more than 20 years confirms an association between abnormal nailfold capillaries, antinuclear antibodies (ANA), and increased mortality compared with those with neither marker.
"The coexistence of ANA positivity and abnormal nailfold capillaries may identify a subgroup with an underlying systemic biological vulnerability that adversely influences outcomes across a broad range of intercurrent illnesses.
