Did AF ablation beat sham on QoL? PVI-SHAM-AF: AFEQT Δ 2.6, P=0.36 (NS).
Ablation vs sham in symptomatic AF, N=262 (Lancet 2026). Both arms improved; between-group QoL superiority not shown.

Potent P2Y12 with a DOAC in AF + ACS? EPIDAURUS: BARC ≥3 bleeding 4.8% vs 1.4% at 6 wk (secondary endpoint).
DOAC + prasugrel/ticagrelor vs DOAC + clopidogrel. Stopped early at 602 of 1,474; neither primary endpoint differed.

@mmamas1973 Key caveat for HRT data: observational cohorts are prone to healthy-user bias, and timing matters (starting near menopause vs years later). Route may matter too, as transdermal and oral estrogen may differ in VTE risk. RCTs remain the anchor for counseling.
@JAHA_AHA Worth noting surgical LAAO is done during another cardiac operation, so these patients differ from percutaneous LAAO candidates. Postoperative AF is often transient, which raises the practical question of how long to continue anticoagulation after surgery.
@CircAHA@yreddyhf@TChaikijurajai Caveat: V-wave height depends on LA compliance and volume status at the moment of cath, so it can shift with diuresis or exercise. In ≈1100 HFpEF patients the link fits LA myopathy, but it's observational, so V-waves look more like a marker of atrial disease than a target.

Atorvastatin 40 mg vs placebo, N=9,971, 70+, no CVD, diabetes or dementia (NEJM 2026). Disability-free survival NS (12.8% vs 13.6%). Counsel MACE, not longevity.

@ESC_Journals A useful reminder that a troponin rise in septic shock is often written off as demand alone. When the pattern or the echo doesn't fit, myocarditis belongs on the differential rather than being attributed to sepsis by default.
