Amazing 😍
🔥SHORTEN 2 RCT
A 7-day treatment course is superior to 14 days for uncomplicated Pseudomonas aeruginosa bacteremia bacteremia, achieving similar clinical response rates with lower antibiotic exposure, fewer superinfections, and fewer serious adverse events
🔥FDA has approved Zaynich (cefepime and zidebactam) for adults with complicated urinary tract infections (cUTIs) caused by designated susceptible microorganisms.🔥 #idxposts@BradSpellberg@IDstewardship https://t.co/WbXvttg5zB
5 billion..yes BILLION people have had a Covid vaccine
If they were anywhere NEAR as unsafe as antivaxxers would have you believe, we should have had a mass global population die off by now
We have not
They are lying
Have a lovely day
I remember when something like this would have triggered an immediate Health Advisory Notice from @CDCgov — alerting clinicians, warning public health officials, mobilizing action.
Now? Crickets.
Public health has been abandoned.
Last flu season, 290 children died in the US. The most since CDC began tracking pediatric flu deaths in 2004. About 9 in 10 were not fully vaccinated.
This season, 71 kids have died so far. Again, about 9 in 10 unvaccinated. Pediatric vaccination rates are at a 15-year low.
Kaine asked the Surgeon General nominee a simple question: does the flu vaccine reduce serious disease and hospitalization? She couldn’t answer it.
This is the cover of @TheLancet
“The destruction… might take generations to repair.”
That’s the damage from RFK — eroding vaccine confidence, weakening public health, and diminishing U.S. scientific credibility.
When a leading medical journal uses language like this, it’s not rhetoric.
It’s a warning the world should take seriously.
New article out on cUTIs and the debate on cephalosporin efficacy. Im still not convinced cephalsoprins are inferior but this is a good read

Will you start using an up front vanco taper for cdiff?
99% probable it is superior to prevent early recurrences versus 14d. Not bad to delay recurrences in sick frail people.
I have changed my practice.
Now we will run an RCT of vanco P-T vs fidaxo

TAPER-V RCT
Initial Vancomycin Taper for the Prevention of Recurrent Clostridioides difficile Infection
In this randomized clinical trial of 256, a 4-week vancomycin pulse and taper regimen *had a probability of 74% to be superior to astandard 2-week pulse regimen in patients a first episode or first recurrence of CDI
*All patients received 14d pulse of vanco (standardized at 125 mg orally 4 times a day at the time of recruitment) →taper (125 mg q12h ×7d,then 125mg q24h ×7d) vs placebo for management of CDI (first episode or recurrence).
Vanco for 10d is obsolete! #IDXposts

The ACIP decision to discontinue the recommendation for a universal hepatitis B birth dose vaccine will cause harm.
As the federal government continues to fail children's health, families can rely on our nation's physicians to continue to put children's health first.

For a long time we have inappropriately been giving abx for liver pts. Here are yet more data. Practice will be changing and more is to come on this. But GI investigators, stop looking at UTIs. They are usually misdiagnosed. Focus on more accurate Dx.
Risk of measles vs risk of measles vaccine
10,000 cases of measles will result in 10-30 dead kids and 2000 hospitalised,
10,000 MMR vaccines will result in 3 kids getting seizure s from a post vaccine fever.
Take your pick.
Not hard is it??
