In this collaborative piece with colleagues across the region, we explore how the new U.S. guidelines for acute coronary syndromes translate into the diverse clinical realities of East and South Asia.
🔑 Key themes:
1. Antithrombotic therapy varies widely—drug access, dosing, and duration differ across countries.
2. Lipid management is aggressive in South Asia (<30 mg/dL LDL-C targets), but high-cost therapies remain out of reach for many.
3. Revascularization strategies reflect infrastructure gaps—ranging from advanced PCI with imaging to fibrinolysis in rural areas.
4. Mechanical circulatory support access is unequal, with reliance on IABP in most settings and limited use of advanced devices.
🌏 The takeaway:
Guidelines are essential but context matters. From aging populations to SES disparities, ACS care in Asia faces unique barriers that demand local adaptation, investment in regional data, and stronger implementation pathways.
