Today marks the 25th anniversary of the Southwest Supermarket Fire in Phoenix, Arizona.
The incident claimed the life of Firefighter Bret Tarver, involved 13 Maydays, and left lasting scars on the members of the Phoenix Fire Department. It was a tragedy that profoundly affected everyone involved—but it also became a turning point for the American fire service.
In the years that followed, Phoenix Fire Department leaders undertook an exhaustive five-year examination of the incident. The goal was not just to understand what happened, but to uncover why it happened. That honest and disciplined review identified critical lessons about risk management, command, communication and accountability on the fireground.
Under the leadership of Fire Chief Alan Brunacini, those lessons were transformed into meaningful changes in how incidents are managed. The work of a dedicated team helped refine and implement new command practices—efforts that ultimately contributed to the development of the Blue Card Incident Command Certification, helping departments across North America strengthen command and improve firefighter safety.
