"We all want personalised, high-quality end of life care, be that for ourselves, loved ones, or our patients. On @ACPDay2024 I would urge BGS members to consider Advance Care Planning as a core part of Comprehensive Geriatric Assessment" #ACPDay2024 https://t.co/fW2XzK2MLL
It can be shocking just how little attention is paid to mobility in hospital. Both at the start: no comment on how the person moves now. Silence on what they were like at baseline. How can we help people “get better” (or home) if we don’t know what better looks like for them?
Congratulations @CkeepKeep for your 25yr service… a fantastic achievement having helped many patients and relatives over the year’s @DCHFT. Very proud of you 👏🏻👏🏻👏🏻👏🏻👏🏻❤️
What a week! My brain is fried and I am really pleased to have the next 3 days off to re-charge my batteries. Looking forward to time with friends over the weekend - fun times! #worklifebalance

@DCHFT#FrailtyConference23 Don’t forget to check for pain! Non-verbal - change of behaviour, facial expressions, physiological effects. Don’t underestimate the importance of pain management (& don’t prod Kate too much when she’s in pain as she may thump you!!)
@DCHFT#FrailtyConference23 The importance of mental health as well as physical health presented by @vikkitweedy Collaborative working is key to meeting the needs of patients
@DCHFT#FrailtyConference23 Great information on the Clinical Frailty Score - important to remember to look at what the patient was like two weeks before admission and not just what you see in front of you
@DCHFT#FrailtyConference23 Really thought provoking information shared by the chaplaincy team. They have “The gift of time” to really listen to patients… and also understand their silence. So important in such a busy hospital environment
What is the point of doing routine 4ATs in the ED?
Most of us will miss delirium and memory problems on brief reviews if we don't do some specific testing.
You might think you're good at just spotting delirium. But you're probably not. (Neither am I)
Today marks 25 years since I stepped through the hospital doors as a qualified nurse. 25 years!! I wasn’t even 25 years old then… It’s been the greatest times, the most challenging times and I have learnt so very many things along the way. But it will always be a privilege ❤️
We have secured £2millon from @NHSEngland to create a larger discharge lounge facility. It will allow us to provide a comfortable environment for patients while they wait to be collected and free up inpatient beds for those that need to be admitted. ➡️ https://t.co/49iOPphawT
Another lovely Sunday, downtime with family, fresh air and cake! Couldn’t be better 😊 and well needed after a manic and stressful week at work last week.
Attended #MCAconference2023 today. Always insightful and thought provoking. Supporting people to make their own decisions if they can and protecting those that are vulnerable.
There are a few simple but important things you can do to help us get your family member, friend or neighbour home from hospital as soon as they are ready. Take a look at our patient guide to hospital discharge: https://t.co/vSEJw3IDWN
