The broader implication of these findings is that plastic surgery should probably stop asking whether a breast implant is simply “smooth or textured.�� Those labels place fundamentally different surfaces into the same biological bucket.
The results here instead demonstrate a continuum associated with quantitative surface roughness. Smooth implants and the 4 μm surface had remarkably similar microbial profiles. By contrast, traditional microtextured surfaces (30 μm) and macrotextured surfaces (>50 μm) showed progressively altered microbial communities, greater Staphylococcus dominance, and an increasing bacterial burden trend.
This supports a more nuanced model of implant biocompatibility. The relationship between surface roughness and clinical outcomes is not linear. Capsular contracture appears to follow a J-shaped curve, with a potential “Goldilocks” zone of minimal microtopography with a favorable foreign-body response without creating microbial niches and chronic inflammatory environments associated with 30 μm and above textured surfaces. Surface design is not just a manufacturing feature, it is a biologically active variable that shapes the implant-tissue-microbe interface.
Importantly, this was an independent academic investigation. The study was supported by institutional and nonprofit research foundations, with no industry role in its design, conduct, analysis, interpretation, or publication. A Johns Hopkins robot known as SRT-H removed a gallbladder by itself with 100% accuracy after watching surgery videos.
It identified arteries, clipped ducts, cut tissue, and even adapted when the visuals changed mid-procedure.
It followed voice commands like "grab the gallbladder head" and adjusted in real-time, just like a human trainee.
It exhibited the expertise of a skilled human surgeon, even during unexpected scenarios typical in real-life, medical emergencies.
The surgery was done on a life-like model, not a real person.
