While you sleep, your brain runs a cleaning cycle that washes out the waste products linked to aging and dementia.
It only works during deep sleep.
And the position you sleep in changes how well it cleans.
Here is what 30+ years of brain research now shows us.
A 2020 review in Brain Sciences, pulled together by neuroscientists at Amsterdam University Medical Centers, walks through three decades of research on this exact system.
What does the research show?
When you fall into deep sleep, your brain cells shrink slightly.
The space between them widens by around 60 percent.
The fluid that surrounds your brain rushes into those open channels,
It washes out the toxic protein that builds up in dementia, called beta-amyloid.
About 500 mL of this fluid moves through your brain every day. Most of the cleaning happens at night.
Here is what the researchers reported:
1. The vast majority of brain waste clearance happens during sleep
2. Side sleeping cleared more waste than back sleeping or stomach sleeping in animal studies
3. Dementia patients spend a much larger percentage of the night on their backs than healthy controls
4. The number of times you change position is not the issue. It is the percentage of the night spent on your back that matters
5. The average person changes position 11 times a night without knowing it
Why does it work?
Gravity moves blood and fluid through your brain differently in each position.
Lying on your back can restrict the drainage of certain veins in your neck.
Side sleeping appears to allow the system to drain as it was designed to.
It is not the only lifestyle factor that changes this cleaning system.
The review also covered:
- Omega-3 from fish oil. Linked to better clearance and lower toxic buildup
- Chronic stress. Associated with reduced clearance and more waste left behind
- Moderate exercise. Linked to improved fluid movement through the brain
- Intermittent fasting. Showed improved clearance in animal models
We all know that sleep is when the body repairs itself.
Now we know what one of those repairs actually is.
Your brain is taking out the trash.
Most of these studies are still in animal models and small human cohorts.
More work is coming.
But the side-sleeping signal is consistent enough that the researchers themselves included it in the recommendation list.
You spend a third of your life asleep.

The Pentagon just lent $620,000,000 to a tiny North Carolina startup called Vulcan Elements. The company is two years old.
It had fewer than 50 employees.
And three months before the deal was announced, Donald Trump Jr.’s venture firm quietly took a stake in it.
Here is the part the administration tried to bury.
Of the dozens of companies the Pentagon was weighing, Vulcan was the only deal initiated by a top White House aide. That aide was Peter Navarro, a close friend of Trump Jr. The order came down to move fast.
One official put it plainly: The call came from the White House. We have to get this done.
Staff worked late nights to push it through in weeks. Deals like this normally take many months of vetting. And when it closed, Vulcan’s valuation jumped from about 200 million dollars to roughly 2 billion.
A windfall for the investors, including the president’s son.
This is public money. Your money.
Routed through the Pentagon to enrich the president’s family and their friends. The Bush administration’s own chief ethics lawyer called it corruption we pay for.
And there is more coming.
A drone parts company Trump Jr. holds a stake in is also under Pentagon review.
This is not a one-off. It is a pattern. The president’s family is treating the federal Treasury like a private bank, and the bill lands on every taxpayer.
