사실 고양이는 태어날 때부터 신장 청소 능력에 결함이 있다.

AIM이라는 단백질은 혈액 속을 돌아다니며 신장에 쌓인 세포 찌꺼기를 표시해두면,
면역세포가 와서 치워가는 구조다.

사람·개·쥐는 신장이 손상되면 AIM이 자동으로 출동해서 청소를 시작하지만,

고양이는 청소부가 출동을 못한다.

고양이의 AIM은 평소에 IgM이라는 단백질에 꽉 묶여 있다.
문제는 이 결합력이 다른 동물보다 1,000배 강하다는 것.
신장이 아무리 망가져도 AIM이 풀려나지 못한다.

이렇게 되면,
찌꺼기가 신장에 계속 쌓임 → 만성 염증 발생 → 신장 조직이 딱딱하게 굳음 (섬유화) → 기능하는 신장세포 감소
→ 만성 신부전 진행
이게 바로 15살 이상 고양이의 80%가 신장병에 걸리는 이유이다.

일본 도쿄대 미야자키 교수팀이 개발한
rAIM 주사제는
묶여있는 AIM 대신, 자유롭게 움직일 수 있는 재조합 AIM을 주사로 보충해주는 방식으로,
임상시험 결과는 다음과 같다.
치료 안 한 그룹: 1년 생존율 20%
rAIM 주사 그룹: 1년 생존율 80~83%

현재 2026년 일본 승인 신청 완료 되었고,
빠르면 2027년 봄 상용화 예정이다.

지금까지 고양이 신장병은 "진행을 늦추는 것"이 전부였는데, AIM은 진행 자체를 멈추는 최초의 치료제가 될 수 있다.
