🔥Some weeks stay with you.

This was one of them.

Our third-year GI fellows are only a few months away from completing their fellowship, and this week I had the pleasure of watching Dr.Mohammed Kawaja and Dr. @DAlshreiif Alsharif successfully perform selective bile duct cannulation during ERCP using my favorite approach: direct cannulation with a sphincterotome.

People often think teaching is about passing on techniques. That's part of it. But it's not the heart of it.

To me, teaching is watching someone struggle, improve, gain confidence, and eventually make the procedure look effortless.

I've always had one simple rule:

Show me your interest, and I'll invest everything I have in teaching you.

Curiosity can't be taught.

The videos below are only 60 seconds long.

What they don't show are the months behind those 60 seconds. Early mornings, difficult cases, countless questions, small corrections, and steady progress.

Seeing that growth is the greatest reward of being a teacher.

Proud of both of you. The best is still ahead.

هناك أسابيع تبقى في الذاكرة...

وهذا كان واحدًا منها.

لم يتبقَّ سوى بضعة أشهر على تخرج زملائنا في السنة الثالثة من برنامج الزمالة، وكان من الجميل أن أشاهد د. محمد خواجة ود. بشاير الشريف ينجحان في إجراء Selective bile duct cannulation أثناء الـERCP باستخدام التقن��ة التي أفضلها دائمًا، وهي Direct Cannulation with a Sphincterotome.

كثيرون يعتقدون أن التعليم هو نقل المعلومة أو تعليم المهارة. هذا جزء منه، لكنه ليس جوهره.

أما أنا، فأجمل ما في التعليم هو أن ترى شخصًا يجتهد، ويتعثر، ثم يتطور، حتى يأتي اليوم الذي يؤدي فيه الإجراء بثقة وهدوء.

ولطالما قلت لزملائي:

"أرِني اهتمامك... وسأبذل كل ما أستطيع لأعلّمك."

فالفضول لا يمكن تعليمه.

المقاطع في الأسفل مدتها 60 ثانية فقط...

لكن خلف هذه الـ60 ثانية أشهرٌ من التدريب، والأسئلة، والتصحيح، والمحاولات، والعمل المستمر.

رؤية هذا التطور هي أجمل مكافأة لأي معلم.

فخور بكما... والقادم بإذن الله أجمل.

🔥Good Journal Club sessions don't end with a conclusion. They end with people still arguing in the hallway.

That's roughly where we landed this month. The paper was the randomized trial pitting Cold EMR against Hot EMR for large colorectal polyps — solid design, clean execution, and exactly the kind of study that looks settled until you start pulling on it.

Then you pull on it.

Cold EMR came out looking safer. Fewer severe events, no perforations. But the primary endpoint didn't actually reach significance, and recurrence nearly doubled. So is it the safer technique or the weaker one? Depends entirely on whose polyp you're holding the snare over. A 25 mm serrated lesion in an anticoagulated patient is not a 35 mm adenoma with high-grade dysplasia, and pretending one trial answers for both is how good evidence gets misused.
That tension — safer signal, worse recurrence, no clean winner — is the whole reason the hour was worth showing up for. Nobody left with a protocol. Everyone left thinking harder about the next case.

Real thanks to #DrBashayerAlshreeif, who didn't just present the paper but stood in front of it and let it get questioned. That's harder than it sounds, and she did it well.

Thanks also to Dr. @Almotasembilla1 Rammal for running the SGA Journal Club the way it should be run — as a place to interrogate evidence, not applaud it. And to Dr. @majidalsahafi Alsahafi, Chair of the SGA Endoscopy Section, whose backing keeps this kind of education alive in our community.

What stayed with me afterward:
The technique doesn't choose itself. The lesion and the patient choose it.
Reading a paper is the easy part. Working out what it actually means for the patient in your room on Sunday morning — that's the part that takes the rest of us a career.
On to the next one.
