This is the colonic appearance of a patient with acute severe ulcerative colitis (ASUC) with deep punched out ulcers resembling Crohn’s disease (this is Mayo 3 ulceration). C diff testing was negative. The patient had failed steroids and infliximab therapy, and eventually required colectomy due to toxic megacolon.
ASUC is important to recognise. I have a low threshold of stopping the colonoscopy early because of the high risk of perforation, avoid retroflexion, and assess extent with CT imaging which helps delineate extent of colitis (and to assess small bowel). Biopsies rarely help (except for CMV) but treatments should never be delayed. Start advanced therapy early and involve surgery at the outset.
