- expect rapid CNS/CVS crash; tube early
- diazepam 2 mg/kg over 30" then 1-2 mg/kg/d
- epi 0.25 μg/kg/min, titrate to SBP ≥90
- NaHCO₃ or 3% NS for ↑QRS
- avoid meds that ↑QT
- watch for ↓K⁺ (shift); don't overcorrect
