Wheelchair evaluation for an almost 3 yo, he’s getting ready to be functional for preschool. Dad suddenly gets quiet and seems uninterested. Reminded me that it’s important for parents to process big changes. Took a break from measurements to let dad hold his son.
Pt presents with 2-5 seizures per day. MD wants to up doseage of Keppra, but PT is already sleeping up to 15 hours a day due to meds. How do you find a good balance between seizure control and waking hours? We want to maximize function, and be awake for PT!
Patient who is non-verbal? We’re trying a new button that is recorded to say “go”. Working on her pushing it after I say “ready, set” followed by movement on the swing. Hoping it will progress to functional communication.
3/3: We are decorating Jayden’s gait trainer with his name on the back and every kid that uses it to help them walk gets to put their footprint on Jayden’s wall. Thank you Jayden and family. ❤️❤️❤️ we are so thankful to help his legacy live on.
2/3: at the end of the tour she says she wants to donate her late sons gait trainer to our clinic. She thought about selling it, by it just didn’t seem right. She hopes other kiddos can use it and Jayden’s equipment can go to good use.
1/2: A mother came in to our clinic yesterday to take a tour. She asked if we used gait trainers. One of my goals for this week was to improve debriefing with kiddos parents and become more familiar with all of our gait trainers- perfect opportunity for learning!
Update on my patient with the night splint tracking chart: he is on track to meet his goal of 150 hours by Halloween. Further questioning revealed mom was nervous the splints would hurt. Spooky how much patient education can help calm everyone’s nerves. 👻
5 year old presents with asymmetrical strength and balance. What’s more important with regards to stair training: biasing the weaker leg to increase reps for strengthening or promoting reciprocal stepping pattern?
At our clinic we take our shoes off when on the mats. I’ve been trying to encourage one patients mom to have her wear orthotics more often. Yesterday, we did our therapy outside and she benefitted from jumping with improved arch support. Sun was nice too!
Accountability: when your patient won’t wear his night splints, so you try making a chart to track hours worn with goals. 150 hours by Halloween gets him a spooky treat! 👻#peds
9 year old toe walker with ADHD: how do I get him to slow down and stop running around the clinic? Yesterday I brought in my swim fins and had him search for hidden fish around the gym. Slowed him down and activated his Dorsi Flexors! Success :)
Imagine you’re watching Netflix laying sideways on your sofa and someone tilts he sofa 45 degreees. You’d probably tilt your head to keep watching, right? Same idea with the football hold for babies with torticollis. Head righting to strengthen CL SCM!
4 y.o. pt presents with toe walking and increased incidence of falls. Is it a ROM deficit, strength deficit, or sensory issue? CP? DMD? This particular kiddo had full range and only toe walked when barefoot, turns out his feet were sensitive!
You’ve heard of MCI, but what about CCD? Canine cognitive dysfunction.
Sx: confusion, anxiety, appetite/sleep changes, 💩 accidents.
Good news: healthy diet, exercise and a predictable routine can help! Shoutout to Stüssy my favorite pup with CCD! @juliehershberg#pt546week13

Headache, seizures, Neuro deficits...How do you differentially diagnose? You need an MRI/CT to differentiate between astrocytoma meningioma, and metastatic neoplasms. #tbt to SNOOPPPP symptoms to know when to go to the ER. #pt546week12@juliehershberg@manjiridahdul
Got a headache? Let’s try to snoop out the cause!
