Educational space .
One thing I’ve learned with sleeve leaks:
Stop trying to close the hole. Drain it.
With endoscopic internal drainage, the pigtails are not there to “seal” the defect. They keep the drainage going internally, the cavity gets smaller, and eventually it heals.
And stent position matters.
The pigtail should bridge the defect, with one end sitting inside the collection/cavity and the other end draining freely into the gastric lumen.
For a large cavity, I usually prefer two pigtails, positioned to maintain good internal drainage rather than simply plugging the opening.
And when EID doesn’t work, there is usually a reason.
A distal stenosis or twisted sleeve you didn’t address.
Another collection you missed.
An external drain still pulling everything outside.
Or simply a dirty cavity that needed cleaning before putting the pigtails in.
For most of these leaks, I prefer double pigtails over covered SEMS. Better tolerated, less migration, and much easier for the patient.
I usually reassess at 4–6 weeks and follow the cavity, not just the size of the defect.
Chronic cavity with a septum? Septotomy.
Acute cavity that is infected and not draining well? EVT can help.
EID and EVT are not competing techniques. Sometimes you need both at different stages.
The question is not always:
“How can I close this leak?”
Sometimes the better question is:
“What is stopping it from healing?”

Six years ago as a fellow at @BrighamGI, we were the first to report on the use of absorbable gelatin sponge as an alternative to cyanoacrylate for the management of bleeding gastric varices (alongside coils).
Excited to see this first-of-its-kind RCT by Ashok Jhajharia published in @GIE_Journal confirming absorbable gelatin sponge's superiority to cyanoacrylate for bleeding GV with reduced re-bleeding and re-intervention rates.

Ending more than 40 years of severe GERD in just 30 minutes using GERDx technology ✨
GERDx is an advanced, minimally invasive endoscopic technique designed to reconstruct and strengthen the lower esophageal sphincter (LES) from within the stomach—without surgery or abdominal incisions. Using specialized sutures at the gastroesophageal junction, GERDx recreates the anti-reflux valve mechanism in a way comparable to surgery, but with significantly less invasiveness.
In this case, the patient suffered from long-standing, medication-refractory GERD with objective evidence of severe acid reflux on pH monitoring and an elevated DeMeester score. Following the procedure, the patient experienced significant symptom improvement and a rapid return to daily activities.
This achievement reflects:
Strong multidisciplinary teamwork
High operating room readiness
A shared commitment to innovation and continuous improvement
My sincere appreciation goes to Dr. Saad Al-Shahrani @DrSaad1986, Medical Director, and the dedicated administrative team for their continuous support in development, quality improvement, and ensuring access to the latest and most advanced treatment options for our patients.

Grateful to the exceptional faculty at KAUH for the opportunity to present live cases. One highlight was a GERDx antireflux procedure with excellent clinical outcomes—the patient was discharged the following day and successfully discontinued PPI therapy.
Endoscopic classification systems are widely available and can help classify endoscopic findings. Adhering to classification systems to obtain reliable results is essential and helpful in guaranteeing transferable reports to others. Read on gutflix https://t.co/CVA7SPo8s6
