From September 9th you will need to use the new attending practitioner MCCD. These have already been sent to hospital sites, GP practices etc. Do not use them before then. The currently used MCCD will not be valid if written on or after September 9th.

Emotional reading the findings. Every bereaved, harmed, family I've met over many years have stated how harmful it is not having a voice,to have to search for openness & transparency, answers, truth. It impacts on every part of life. There has to be a kinder way for all.
