How is it that insurance companies in California are compelled to cover the most insane trans surgeries in the least stable patients?

Like a psych patient with a history of psychosis getting approved for a "vaginoplasty."

Great reporting from @a_chrisbray shows how it's done.

When a surgery is denied by insurance, the patient can appeal the denial to an Independent Medical Review (IMR).

When an IMR overturns the denial, insurance is mandated to cover the surgery.

Instead of getting surgeons to review these cases, California stacked their IMR with radical psychiatrists to overturn all denials. Those psychiatrists use WPATH to justify the "medical necessity" of the surgery.

Bada-Bing Bada-Boom! All trans surgeries are now covered as "medically necessary."

If that wasn't bad enough Californians can see how much they're getting screwed since all the appeals are publicly available on their website (see screenshot).
