✅المريض هذا لما جاء بـ Decompensated Heart Failure عطوه في الطوارئ loop diuretics ومنها Furosemide عشان يتخلص من الماء الزايد اللي تراكم في جسمه، وهنا تبدأ القصة.

الـ loop diuretics تشتغل على الـ thick ascending limb في الـ Loop of Henle داخل الكلية وتوقف امتصاص الصوديوم والكلور والماء، بس المشكلة إن الكلية عندها transporter اسمه URAT1 مسؤول عن إخراج الـ Uric acid في البول، لما تيجي الـ loop diuretics وتخلي الجسم يفقد كميات كبيرة من السوايل يصير عندنا شيئين يضربون المريض مع بعض، الأول إن الدم يتركز وتقل كمية السوايل فيصير عندنا Volume depletion وهذا يحفز الكلية ترجع تمتص كل شي زيادة بما فيها الـ Uric acid بدل ما تطرده، والثاني إن الـ Uric acid نفسه يصير يتنافس مع الـ diuretic على نفس الـ transporter في الكلية ف يفوز الـ diuretic ويطلع بره والـ Uric acid يفضل محبوس في الدم، النتيجة إن مستوى الـ Uric acid في الدم يرتفع وهذي الحالة نسميها Hyperuricemia، ولما يرتفع الـ Uric acid فوق طاقة الدم على إذابته يبدأ يترسب على شكل بلورات اسمها Monosodium Urate Crystals وهذي البلورات تحب تتجمع في أبرد مكان في الجسم وأبعده عن القلب وهو مفصل إصبع القدم الكبير اللي نسميه الـ First Metatarsophalangeal Joint، لما تتجمع البلورات هناك جهاز المناعة ما يتركها، الـ Neutrophils تحس فيها كأنها جسم غريب فتهجم عليها وتحاول تاكلها بالـ Phagocytosis، لكن البلورات هذي قاسية وتخرش الـ Neutrophil من الداخل فتتفجر وتطلق كل المواد الالتهابية تاعتها زي الـ IL-1 والـ TNF وهذا يسبب التهاب حاد مفاجئ في المفصل يصحى منه المريض من النوم ويلاقي إصبعه أحمر منتفخ ساخن كأنه اشتعل نار، وهذي هي الـ Acute Gouty Arthritis.

عشان كذا المريض اللي يجي بـ Heart Failure ويعطونه loop diuretics لازم الطاقم الطبي ينتبه لمستوى الـ Uric acid عنده ويحذره إنه قد يحس بعد أيام بألم في إصبع قدمه وما هو شي جديد بل هو side effect معروف للعلاج.

Understanding the Doppler angle is key to accurate velocity measurements.

Doppler ultrasound gives the most accurate blood flow velocity when the ultrasound beam is aligned parallel to the direction of blood flow (angle = 0° or 180°).

As the Doppler angle increases, velocity is increasingly underestimated. At 90°, measurement becomes unreliable.

Always aim for parallel alignment to capture the maximum Doppler frequency shift and avoid error.
