There is a cholesterol number you inherited from your parents, you only have to measure it once in your entire life, and almost no man over 40 has ever been handed it.
In April of this year, eleven major heart and medical organizations published a new cholesterol guideline in Circulation. Buried inside it is one of the most useful sentences written for men in 2026: every adult should have lipoprotein(a) measured at least once in their lifetime.
That came in as a Class I recommendation. That is the strongest wording a guideline issues.
Here is what men get wrong. You assume your cholesterol panel already covered this. It did not.
A standard lipid panel gives you four things. Total cholesterol. LDL. HDL. Triglycerides. Lipoprotein(a) is a separate order. It is not in the default bundle. If nobody checked that box, you have never seen your number.
Now the biology, because this is not just another line on a page.
Lipoprotein(a) is an LDL-like particle with an extra protein wrapped around it. That extra protein does three things. It makes the particle stickier inside the artery wall. It makes it more inflammatory once it lodges there. And it interferes with your body's own clot-clearing machinery.
So it pushes plaque and it nudges the clotting side at the same time.
And the artery lining it does that to is the same lining that runs your erections. Same endothelium. Same nitric oxide. Erections are vascular, not age-based — so anything working on that lining is working on both.
Roughly one person in five carries an elevated level. Risk starts climbing around 125 nmol/L, and observational data has it roughly doubling north of 250.
Why only once? Because it is almost entirely genetic and it barely moves across a lifetime. Diet does not meaningfully change it. Training does not meaningfully change it.
And that is exactly where men misread it. They hear "you cannot change it" and conclude the number is useless.
That is backwards.
A fixed number is a starting position. It tells you how hard everything you CAN move needs to be managed — blood pressure, waist, A1c, LDL, training, smoke.
A man with a high number and a tight lifestyle can do well. A man with a high number who never knew it, and let the other five slide, has been carrying a load he never agreed to. That creates biological drag for twenty years before anything announces itself.
The body keeps score whether or not you ever read the scoreboard.
Tools. Today.
Ask for it by name at your next blood draw. The sentence is: "I would like lipoprotein little-a added, once, for lifetime risk." It does not require fasting. It does not require repeating.
Ask for the result in nanomoles per liter, and write it down somewhere you will still find it in ten years.
If it comes back elevated, that is a reason to get everything modifiable tight — and a reason to mention it to your brothers and your adult children, because it runs in families.
If it comes back normal, you are finished with it permanently. One less unknown on the board.
And do not let a high number become a reason to buy something online. It is a reason to book a real conversation.
One blood draw. One time. Stop with the excuses.
