1. Footballer
Anterior ankle pain on powerful kick, sprint & inclines
When very irritable, gets an uncomfortable 'flicking' sensation
Pain reproduced on dynamic lunge & resisted dorsiflexion
Tender over tibialis anterior & into anterior joint recess
MRI shows inflammation anterior ankle joint line
POCUS video - large dorsal talar ostophyte irritating TA tendon on active DF / PF
So, 2 problems - 'footballers ankle' plus compression of TA tendon under load & through ROM
US guided injection to anterior ankle impingement zone, podiatry & physio input before RTP
I remind my patients this way:
“Tendons need something to do: Not too much- but also not too little”. 💪
I also say, “The muscle wants to be strong and springy- that helps the tendon to recover”.
Consider intermittent bracing to tolerate reload.
More tendons are “injured” from inactivity than overuse.
Sedentary people develop tendinopathy as often—if not more often—than those with overuse injuries.
Inactivity, inflammatory mediators and poor metabolic health weakens tendons, making them more prone to pain and dysfunction.
More people are sidelined by stillness than by movement.

Every time I see somebody with a nosebleed doing this wrong I feel the need to get this message out:
If your nose is bleeding, DO NOT TILT YOUR HEAD BACK. This just makes the blood drip down the back of your throat and you will then swallow and/or choke on it. Pinch the soft part of your nose and TILT YOUR HEAD FORWARD. Please tell all your friends about this.
@DrJN_SportsMed You are not crazy. I saw a hiker with anterior hip pain. Had visited a rheumatologist (no xray), and a sexual health clinic. All said vaguely ‘you have a soft tissue condition’. Begged for an xray at an urgent care clinic. Mod-Severe bilateral OA hips.
