I saw gallbladder hepatization for the first time yesterday.

GALLBLADDER HEPATIZATION refers to a condition in which the gallbladder loses its normal anechoic (black, fluid-filled) appearance and instead shows liver-like echotexture on ultrasound, making it resemble adjacent hepatic parenchyma.

📍​ Ultrasound Features:
1.​Echogenic Content Filling Gallbladder
2. ​Liver-like Echotexture (Hepatization)
3. ​Reduced or Absent Posterior Acoustic Enhancement
4. ​Poor Wall–Lumen Differentiation
5. ​May Show Internal Debris / Septations

​This sometimes makes the gallbladder difficult to identify on ultrasound because its contents mimic the echotexture of adjacent liver parenchyma.
This phenomenon is termed the "camouflage effect."

📍​ Doppler Findings:
1. ​No internal vascularity - suggests sludge, pus, or debris
2. ​Increased wall vascularity - indicates inflammation (cholecystitis)
3.​ Internal vascularity - suspicious for malignancy

📍 ​Differential Diagnosis
1. ​Tumefactive sludge
2. ​Empyema of gallbladder
3. ​Gangrenous cholecystitis
4. ​Gallbladder carcinoma

In fetal growth restriction, the baby’s brain is “spared” by getting extra blood.
