I saw gallbladder hepatization for the first time yesterday.
GALLBLADDER HEPATIZATION refers to a condition in which the gallbladder loses its normal anechoic (black, fluid-filled) appearance and instead shows liver-like echotexture on ultrasound, making it resemble adjacent hepatic parenchyma.
📍 Ultrasound Features:
1.Echogenic Content Filling Gallbladder
2. Liver-like Echotexture (Hepatization)
3. Reduced or Absent Posterior Acoustic Enhancement
4. Poor Wall–Lumen Differentiation
5. May Show Internal Debris / Septations
This sometimes makes the gallbladder difficult to identify on ultrasound because its contents mimic the echotexture of adjacent liver parenchyma.
This phenomenon is termed the "camouflage effect."
📍 Doppler Findings:
1. No internal vascularity - suggests sludge, pus, or debris
2. Increased wall vascularity - indicates inflammation (cholecystitis)
3. Internal vascularity - suspicious for malignancy
📍 Differential Diagnosis
1. Tumefactive sludge
2. Empyema of gallbladder
3. Gangrenous cholecystitis
4. Gallbladder carcinoma
In fetal growth restriction, the baby’s brain is “spared” by getting extra blood.
The middle cerebral artery (MCA) widens → resistance falls → PI becomes low on Doppler. This is the classic brain-sparing sign.
I have attached the normal high resisitace waveform you expect to see in the MCA. You can compare it with the low resistance waveform in the original question.
