Endoscopy reveals reflux esophagitis, and imaging shows the gastroesophageal junction and upper stomach sliding above the diaphragm. Esophageal manometry demonstrates preserved peristalsis.

During surgery, the stomach is returned to the abdomen, the diaphragmatic opening is repaired, and the gastric fundus is wrapped completely around the lower esophagus.
