I have never seen tb bacteremia but I think tb > mac bloodstream infections in endemic countries, but can happen at cd4 < 100 whereas mac usually only when cd4 < 50. Although I am sure there is an exception for everything. I agree with others re: tx both until PCR back
@BradSpellberg@Infected_Terran I can’t see the whole article but I don’t know what they mean by “robust” pleocytosis.
I think the point of the star and the CID article was that we miss hsv a lot, and we can’t go by neg hsv pcr or by no pleocytosis to rule it out if pretest clinical suspicion is high

It was this article. And yes, it was shocking!
But it was right after STAR encephalitis review. This is was focused on encephalitis, not meningitis (and they largely discussed hsv)

@AnilMakam Well that was the whole debate. I am on #teamdoesnotmatter but pple feel very strongly about it. Like I said, I hope it translates to debates on other topics but it does require the more open and fearless folks to engage and participate
