Is the mechanism of developing this understood?
My understanding is that there is an association with recurrent intracameral air or gas use.
Do you know if the patient required rebubbling post-operatively?

Bonus points for:
- Most common bacteria associated with the condition?
- Medications that can cause the condition?
- Other system associations that should be asked about on systems review?
The central scar and iris adhesion made it impossible to complete the case without removing the cornea.
The way around this was:
- 8.25 mm trephine
- Careful dissection of the iris from the cornea
- Sacrifice of a small area of iris that was firmly adherent

@kr156 We typically do angio guided cautery. Identify the feeding vessel, create a tract with a 30G needle at the slit lamp pre-op, use an electrolysis probe to cauterise. Sometimes with subconj avastin.
Do you think mitomycin has added benefit over this technique?
@simononaraigh Nice one Simon. Have you seen a case before? Particularly difficult to suture into the thin ectatic tissue during penetrating keratoplasty.
@HammadNasti Haha, no such thing as stupid guesses! This is a case of keratoglobus. The entire cornea is ectatic and thin rather than the inferior cornea as in keratoconus. We did a PK/phaco for this patient which was particularly difficult - suturing into such an ectatic cornea.
