me personally, id start an adrenaline infusion and give lasix, do a pocus, check the ivc, if still some room, 200ml normal saline bolus.

can consider a stat dose of albumin.

@SeeFisch if the patients have ild, we use cyp here for 6 months then switch to mmf.

sometimes due to gut involvement, they dont tolerate the mmf so we use mycophenolic acid.

we've used cyp in one of our patients with progressive git involvement which seemed to help.
