You don’t place teeth where the bone is. You place teeth where the teeth belong.

That simple concept represents one of the biggest shifts in modern implant surgery.

Too often, treatment plans are dictated by existing anatomy. The better approach is to begin with the desired restorative outcome, then determine how to safely and predictably support it.

Sometimes that means bone grafting.
Sometimes it means angled implants.
Sometimes it means pterygoid or zygomatic implants.

The objective isn’t simply to place implants. It’s to restore the patient with function, esthetics, and long-term stability in mind.

Digital planning allows us to start with the final restoration and work backward—making every surgical decision more precise and more intentional.

Implant complications are among the most difficult situations we encounter in dentistry.

When they occur, our first responsibility is to care for the patient—not to assign blame.

But once a treatment complication has been addressed, an important question remains:

Who assumes the financial responsibility?

Is it the surgeon?
The restorative doctor?
The patient?
Or is the responsibility shared?

There is no universal answer. Every situation depends on the specific clinical circumstances, the treatment plan, informed consent, communication among providers, and, most importantly, doing what is fair and ethical for the patient.

Not every patient needs a bone graft for a full-arch implant restoration.

One of the most common questions I hear from patients considering All-on-X treatment is:

“Will I need a bone graft?”

The answer is: not necessarily.

Advances in implant surgery have given us more treatment options than ever before. In carefully selected patients, techniques such as pterygoid implants may allow us to provide a stable, long-lasting foundation for a new smile while avoiding more extensive bone grafting.

That doesn’t mean this approach is right for everyone. Every patient is unique, and the best treatment begins with a comprehensive examination, advanced 3D imaging, and a personalized surgical plan.
