Beyond LDL: Why ApoB is the Real Metric Indians Should Watch
If you are only looking at your "Bad Cholesterol" (LDL-C), you might be missing half the story. For the Indian population, which often deals with the "thin-fat" phenotype (high body fat and high triglycerides despite a normal BMI), Apolipoprotein B (ApoB) is a much more accurate predictor of heart disease.
Why ApoB Matters
LDL-C measures the amount of cholesterol, but ApoB measures the number of atherogenic particles.
Think of it this way: LDL-C is the amount of cargo, but ApoB is the number of trucks on the road. It’s the number of trucks (particles) that causes the traffic jam (plaque) in your arteries.
ApoB Target Values for Indians
The Lipid Association of India (LAI) recommends stricter targets because of our higher genetic predisposition to early-onset heart disease.
1. Extreme Risk (Heart disease + Diabetes/CKD):
< 50-55 mg/dL
2. Very High Risk (Known heart disease or stroke):
< 65 mg/dL
3. High Risk (Diabetes, hypertension, or strong family history): < 80 mg/dL
4. Moderate Risk (Younger individuals with few risk factors): < 90 mg/dL
5. Low Risk (General population): < 100 mg/dL
