Zachariah DeFilipp discusses results from a comprehensive, integrated analysis of two clinical trials, evaluating axatilimab in patients with #cGvHD and related BOS (N = 117). The NIH lung response rate on study was 38.5%, with a median time to first response of 2.6 months. Response rates were consistent across baseline lung involvement, including 37.1% vs 40.5% in patients with baseline FEV1 >39% vs ≤39%, respectively, and 37.0% vs 40.2% in patients with NIH lung scores of 3 vs <3, respectively. Among 75 evaluable patients, 32.0% had ≥5% absolute improvement and 20% had a ≥10% absolute improvement in FEV1% from baseline.

Zachariah DeFilipp shares findings from an analysis of two prospective clinical trials and a long-term follow-up study evaluating belumosudil in patients with sclerotic skin and fascial #cGvHD (N = 159). Belumosudil achieved clinical responses across skin and joints/fascia, with ORRs of 28.3% in skin and 62.3% in joints/fascia. Median TTR was 12.0 weeks in the overall cohort. Multivariable analysis showed that higher baseline Lee skin symptom burden was associated with higher risk of treatment discontinuation (HR, 1.89, p = 0.0167), while higher baseline Lee skin and joint composite symptom burden was associated with lower QoL (p = 0.0003). Clinical severity rating correlated with corticosteroid dose reduction (p = 0.0006).

The NIH lung response rate was 38.5%, with a median time to first lung response of 2.6 months. NIH lung responses were 37.1% and 40.5%, for patients with baseline FEV1 >39% and ≤39%, respectively; and 37.0% and 40.2% for patients with baseline NIH lung symptom scores of 3 and <3, respectively. The safety profile of axatilimab in this subanalysis was similar to that of the full study populations.

Yi-Bin Chen, @harvardmed, provides insights into the role of MRD testing in post-HCT maintenance for AML. He discusses various maintenance treatment options, including FLT3 inhibitors, HMAs, IDH1/2 inhibitors, and menin inhibitors, and their importance in preventing post-HCT relapse. He also highlights the need for uniform, highly sensitive MRD assays, such as mutation and host-specific assays, as well as individualized blood-based MRD assays that could help identify patients for maintenance therapy, guide pre-emptive strategies, determine optimal treatment duration, and guide the use of cellular therapy.
