When an easy RV revision seemed to never end. Occluded left subclavian, tunneling of a new RV from left, then discovered an insulation defect of the LV with as well new tunneling from left. CS electrode not extractable, but luckily, the target vein could take another lead in.

Several inappropriate ICD therapies (ATP) inducing VT / VF with eventually effective schock. Problem caused by an intermittent undersensing of the atrial lead. Peri-incisional (post-CABG), RA flutter, today 2:1 conduction, has been ablated. @BSC_EU_Heart @BSCCardiology
