I usually reply that the brain is highly relevant but not the whole story.
The second part to address is the "disease" part though. Maybe that is more the culprit of this being a loaded question (more than necessary imo)

No single measure (or domain) acts alone: individual associations are modest, largely inseparable, and often reciprocal over time across measures.

Interestingly, >half of the variance explained by polygenic scores overlapped with lifestyle and environmental factors.
